Print downloaded application form, complete the form.

Then scan the completed form attached with the following documents:

WASC/G.C.E. O'Level/TC II Certificate or equivalent
Two recent passport-sized photographs
State of Origin Certificate
Declaration of Age
Scanned payment notification of N10,000 Application fee
