Then scan the completed form attached with the following documents:

Professional Diploma or ND Certificate or equivalent in relevant fields
Copy of Diploma Transcript
Two recent passport-sized photographs
State of Origin Certificate
Declaration of Age
Scanned payment notification of N10,000 Application fee
